Ramona Rehabilitation and Post Acute Care Center is a 5-star skilled nursing facility dedicated to providing exceptional care and rehabilitation services to our residents. We are seeking a motivated and dedicated Physical Therapist to join our rehabilitation team and contribute to our mission of enhancing the quality of life for our residents.

WE ARE OFFERING A $8,000 SIGN ON BONUS. DETAILS DISCUSSED AT TIME OF INTERVIEW.

Key Responsibilities:

• Develop and implement individualized treatment plans to improve residents' mobility, strength, and overall physical function.
• Perform comprehensive evaluations and assessments to identify physical therapy needs and goals.
• Collaborate with interdisciplinary teams, including occupational therapists, speech therapists, nurses, and physicians, to ensure holistic care.
• Provide education and training to residents, families, and caregivers on therapy techniques and strategies for optimal recovery.
• Maintain accurate and timely documentation of therapy sessions, progress notes, and discharge plans in compliance with regulatory standards.
• Utilize evidence-based practices to deliver effective rehabilitation services.
• Foster a supportive and motivating environment that encourages residents to achieve their highest potential.
• Participate in team meetings, quality improvement initiatives, and ongoing professional development activities.

Qualifications:

• Licensed Physical Therapist (PT) in CA (or eligibility for licensure).
• Doctorate in Physical Therapy from an accredited program.
• Experience in skilled nursing, rehabilitation, or geriatric care preferred but not required.
• Strong interpersonal, communication, and teamwork skills.
• Commitment to providing compassionate, patient-centered care.
• Knowledge of relevant documentation and compliance standards, including Medicare regulations.
